Current Manga: Well we saw some people in soul society just vanish and we get to see two other vizard memeber the guy is former 9 division and the girl is former lieutenant and we saw a boy who is vice-captain for 9 division right now

Because he is the 3rds squad replacement which has no captain since ichimaru gin left sereitei.There is a new filler arc cause the anime is close to the manga and in order to allow Tite kUBO to publish more material.

Current manga: Urahara has been promoted to be a captain if that's what you mean.Bonus question: Hiyori have to follow Mayuri's order because in the research division, Mayuri is second in command, and Hiyori just the laboratory head. So, Mayuri outrank her.
