The first Silent Hill. I played that when I was fairly young ... and seeing the child demons and hanging corpses creeped the hell out of me. The ghost baby of the school, the nurses of Brookhaven, the sudden darknes in some areas, and the screaming sewer monsters made me shudder and have nightmares for weeks.

There was a game called Thrill Kill that was supposed to be released for the PS but never was. You're able to download it and play it though now.

The Fallout series... It's not often you get to see someone get their side blown away, exposing their innard, then their kneecap blown off, and finally their shoulder, knocking them down. Or seeing someone decapitated. Or less-than-comically electrocuted. Or melted into a puddle of goo.But then, with an isometric perspective, there was a lot left to the imagination. But even so, it was more brutal than most games I've seen nowadays.

Phantasmagoria, this video shows it all. Considering it was made in 1995, it's VERY brutal:

I think Condemned was pretty brutal, mostly because you are usually forced to beat grisly people down with blunt objects. or sharp ones...

Manhunt has to be the most brutal game as far as gameplay is concerned - different levels of execution, wanton violence, loads of gore, everything a budding psycho could want. I had a friend who got scared when someone left it out of its case overnight, fearing that the CD would kill him in his sleep. As far as difficulty goes, I have to say Devil May Cry 3. I still haven't beaten Virgil the last time on DMD mode, making it the only game I have never finished due to difficulty issues.
