It was Johan Santana in 2006. I didn't understand the last part but I think the answer is"In general, when one refers to a player as having won a Triple Crown, they mean that the player led his own league in the three categories. A superior but less frequent circumstance is the Major League Triple Crown, wherein which the player leads the entire major leagues in each of the three categories and not just his individual league."

It's Johan Santana, and he won it in 2006And the triple crown for hitters consists of leading the league(at the end of season) in home runs, runs batted in, and batting average.

After you beat the game once without warping, the game will save and take you to the title screen. Press B and you will be taken to World 9 with only 1 life despite the number lives you earned before.

I believe it's beat the game without warping and then after it saves and you get the saved game confirmation, hit B at this screen. You don't get the title screen unless you did warp.

+50I would have just accepted "beat all the levels from 1-1 through 8-4 without warping", but the first person's reply about going to the title screen was completely inaccurate, so I couldn't give the points. The Races are the Kentucky Derby at Churchill Downs in Louisville, Kentucky. 2nd Preakness Stakes at Pimlico Race COurse in Baltimore, Maryland.Last is in Belmont Stakes at Belmont Park in Elmont, New York.
