I think the Manhunt Series and the developing Project Origin will be the most brutal game. Saw trailer of Project Origin and blood was every where, slow mo coming into your face and fast with weird creatures getting owned by Alma.

Mad World is because you can literally do anything with your surroundings to dispatch your enemies. If you don't have anything around you, you just take out a chain saw. It isn't out yet, but you can look for it on google.

The most brutal game I've ever played would probably be Halo.Yeah.I don't play these kinds of games very much. :|

Gotta go with a classic here. Mortal Kombat, Sub-Zero's fatality. It was bad enough that it got the game nearly banned in the US, Nintendo of America had to change it, etc. The entire game is flat-out brutality.

Ninja Gaiden 2. it was seriously built for it, just press Y on a decapitated enemy for instant gore.Halo is pure tame in comparison
