once i played twisted metal 2 for about 5 hours, then i took a break. But my brother who was playing with me at the time, suddenly dropped his controller yelling in pain and clutching his thumb lol (thumb cramp ftw) personaly tho, i have only ever had my eyes hurt.

Guitar Hero 2 has made me pretty sore in the past, haven't played 3 yet but I find Jordan to be enough of an endurance test, let alone Through the Fire and Flames

Most of the time my eyes start to burn after several hours of game play but I got eye drops to fix that. Playing drums on Rock Band allways gives me the hip/butt gramp of death....

Rock Band, the kick pedal, Green Grass and High Tides, difficulty-Medium...which is sad cuz I'm a drummer in real life, but the last 4 minutes were really wearing down my leg...I really could have used a double bass...

I have arthritis in my thumbs at the moment due to playing my old gameboy color. You have to slam those buttons sometimes to them to work.

I accedentally hit my younger cousin when I was playing LoZ TwP on the Wii.... I felt really bad about it (and still do) but it's kinda funny...(I feel like a bad person for saying it). I was just so into the game! It was that stupid bridge part...Oh! I also almost dented my celing playing bowling on the Wii...controller sliped..

Sometimes when I play emulator ROMs I get a soreness in the back and butt from sitting for over 2 hours. =\ I think I'm going to turn into a hunchback...

My thumbs get sore from playing my DS too long and my sholder gets sore from playing computer games becaues my desk is too high.

When ever i play FPSes on my PSP, my thumbs ALWAYS gets sore, especially when playing socom FTB1. Also, playing Wii gets me sore also for obvious reasons....

I used to get blisters on my thumbs when playing fighting games when I was younger. 
I'd just spam the motions for a Hadouken from the moment I see "READY?" on the screen. It used to crack up my older cousin who was a Street Fighter veteran.

Usually my thumbs get sore if I haven't been playing a counsel game for a while and I spend a day playing it. My fingers will get sore if I have to use the shoulder buttons a lot. DDR will also get my feet sore if I play that too much.
