I always go with the default names because that's their real names...and I don't have enough imagination to come up with a cooler one.

I usually name the main character Aeon. Aeon Phoenix if a surname is required or fits.The exception is games where the main character has a definitely established name (like Link in the Zelda series) where I will always call them by their canon name.

If there is a default name, I stick with that. But with everything else...most often they're random Japanese names. 
Or...they're my character's names. Which are also Japanese. I never stick to one name...though I have repeated names before. ^ ^ Only when I started playing for the first few months I just used my own name. But then when the complication that most of the main characters were guys...yeah.

I almost always go for the default name for my characters. I"m not particularly creative when it comes to choosing names.

Depends what game I'm playing - generally if it's Harvest Moon and stuff like that, I name the dude Thomas just because it's my favourite name for a guy so I use it often. If it's a more fantasy like setting like Final Fantasy Tactics Advance or something like that I have fun choosing random names off a generator and then messing up the letters to make it sound exotic. 
I'm so weird.In Ragnarok Online, my main character was always Lunaria which I got off a random flower website... it's the latin name for some flower/plant. I chose it also because it sort of sounds like the moon.
