I always hated getting to the Great Palace in Zelda 2. When I was younger it seemed like it took all of my lives just to make it there, and once I did, I had a couple of hit points left, and that didn't last long at all. One shot in the palace, BOOM, dead, back to beginning you go.

On Modern Warfare 2 Multiplayer. We were playing on Afghan, Ground War Team Deathmatch. We were losing and the score was 6300-9800. I was on a roll and only needed one more kill to get the Tactical Nuke. Most of my classmates (Including girls) were watching me, and they were really impressed I got that far. I saw the enemy's score on 9900, I then ran around until I got to the top of the cliff and I saw someone, I jumped and I killed him, but forgetting my third perk wasn't Commando Pro, I died when I fell, but still earned the Tactical Nuke. Respawned, I was gonna press the right button, but I died. Of a headshot from a mile away. Really embarrassing.

During Majora's Mask I fell short a few minutes while trying to complete Stone Tower stuff. I had to restart the whole level again.... and I just wasted 140 minutes of progress.

quickly hitting A and using a move that i totally did not want to use, as i was battling the champion, losing the battle. so annoying.
