Wow. This question brings back memories. There's three main things that, even to this day, I still fear in video games.
1. Enemies that follow you. They're relentless, they're often unkillable, and they never, ever stop coming. The Boo Buddies are the classic example. Took me a few years to get over them, but it was a personal moment of awesome when I went 'screw you guys. I'm running past you even if you wanna follow me. Taste my dirt, death bags!' They still kinda scare me, especially when they show up from nowhere or the big ones come along, but at least normally I'm like 'ho hum. Another Boo Buddy. Still shy after all these years.'
2. The oncoming wall of doom. I'm talking lava, deadly water, or other obstacles that keep you running, trying to avoid enemies and certain death. I always panic when I have to go through some breakable walls because they slow you slow down then and if I'm too slow, I'm dead.
3. Enemies that eat you. Sometimes they're like the #1 enemy where they chase you, but other times they aren't. But I have a fear of The Nothing ever since I watched the Neverending Story movie as a little kid and it's kinda like being eaten alive (by a big blob of eternal oblivion), which is another big fear, so a big giant enemy that suddenly shows up and eats up your character gets a girly scream from me.

I got startled when I played through Skyrim the first time. It wasn't even anything scary, really. I was wondering through the woods and I hear this growl. I turn around and BOOM! wolf in the face. I jumped out of my chair and screamed like a little girl.
I'm not proud. ![]()

Doom 3 did that quite a bit... So many jumpy parts... The part in the dark where you follow the guy with the lantern... The spider demon ambush actually caused me to shut the game off for a while.
The latest game that scared the hell out of was Outlast. That game is TERRIFYING.

A lot of games have really just scared the crap out of me, and not all of them are horror titles, but playing Outlast with the Rift by far takes the cake.

There were some nice, tense moments back when I was playing Bioshock, and the Resident Evil games were pretty hair-raising back when I was a kid. But other than that, not really. Haven't gotten 'round to playing Amnesia yet, though, I hear that's a big one for horror stuff.

Yes, since I can be somewhat of a jumpy person. First experience that comes to mind was when I was at a friend's place playing the demo of half life on pc. The scene were the scientist is desperately banging on the window to be rescued from his inevitable fate.
Edit: Since this is actually about consoles and not PC games, I should probably try and provide a more on topic response.
Hmmm I found a few of the vaults in Fallout three and New Vegas to make me jump a fair amount. Some of the back story was quite creepy.

I've played many games that have scared me. Pretty much anything game that has an enemy or force in it that can't be defeated startles or scares me. The first game that scared me however was Resident Evil for the PS1.... Those dogs! Now I dislike all zombie dogs.

I recently just played that game outlast for the ps4 and it literally beat out any other potentially "scary game".

Half-Life scared me. Sounds lame, I know. There were moments when you go down a diagonal elevator platform and crabs are sliding down after you, I got so jumpy and panicky it was hilarious.
Doom 3 made me cack my pants too.
Generally though I don't really get scared with games, only excited or full of adrenaline.
