Without doubt its Super ghouls and ghosts the SNES and GBA versions are both the most difficult games ive ever had the dis-pleasure of playing.

I can't argue with you there that super ghouls and ghosts is incredibly hard (Maximo is much more watered down compared to this grand daddy... or are they the same person?). When he jumps, you almost loose all control and the later stages are all platforming. Thankfully though, during this time, he has an added upgrade to his armor which enables him to powerup his weapons and another upgrade to charge current weapons (Am I seeing the origins of the ability to charge different weapons in Megaman X?).But the most hardest game than super ghouls and ghosts is none other than ghouls and ghosts. No powerups but can acquire different weapons. No double jump so the platform stages are more difficult. Enemies respawn continiously and almost all of them are cheap (As in "YOU CHEAP M@$@F#%K. I'LL SEE YOU IN HELL B#%^H").
